Who we are
The Tissue Viability Service supports patients with wounds such as bed sores, leg ulcers, diabetic wounds, and other wounds that have failed to heal over time.
Our specialist nurses are trained in advanced wound care. They provide specialist advice and support to health and social care professionals treating patients with a non-healing or chronic wound.
What we do
If you have a non-healing or slow healing wound, with your consent, your healthcare professional will contact us for specialist advice and support.
One of our specialist nurses will arrange an assessment appointment to find out more about your wound, your current treatment plans and to discuss any concerns or queries you might have. You will be seen in your usual place of treatment (eg home, clinic or GP practice).
You will be given information about the best treatment for your wounds, including tips about what you can do to help your condition, and together with your healthcare professional we will develop a wound care plan.
How to access the service
You can be referred by your healthcare professional eg Community Nurse, Podiatrist, GP or Practice Nurse, Care Home Nurse and other Allied Health Professionals using the Tissue Viability referral form (Please note: this form is for use by health professionals only).
